Forklift Preshift Inspection Log
The Forklift Preshift Inspection Log keeps all records on the truck including accident reports, near miss reports, PMs performed, break down requests and 624 preshift inspections. If the truck requires maintenance there is a forklift repair request form that can be filled out to alert management of the mechanical failure.
